The Heart Failure Program at Conemaugh Memorial Medical Center works with each patient and their doctor to help monitor their salt and fluid intake, diet, medications, and activity to lead a healthy life after heart failure. Patient Derrick Holloman said, “It’s a one-on-one program. They call you every day to see if you’re breathing right, ask how you are feeling, and check on your medicine. They are on top of your daily health.” Now he has the energy to enjoy life again with his family.
